REQUESTING THERAPY SERVICES
If you are interested in therapy sessions an initial consultation will determine next steps. All therapy sessions include an assessment, the development of a treatment team, the development of a treatment plan, and ongoing case reviews.
- The initial consultation involves the prospective client filling out a request for services, this involves
- Contact Information the name of the client, daytime phone number, email and preferred payment method.
- A verbal or written request for services the request explains the client's current situation and the reasons for requesting services
- The perspective client is also asked to complete the referral intake form this includes listing present and past psychiatric treatments, medical conditions, listing of current medications, previous and present substance use and substance treatment.
- Once all the information has been completed and returned the perspective client will be contacted within two working days to schedule his/her appointment and arrange a payment method.
- Once a payment has been received the client will be issued a receipt for services. He/She may submit this receipt to their insurance agency for possible reimbursement.
All services are individualized and specific to support your success. Services are provided in a structured and planful manner. Allowing "YOU" the advantage. Services allow for continuous support in varying degrees.
